The Fort Worth Country Day School Athletic Facilities are available for practice use by non-school sponsored teams after school hours provided that the group is comprised of at least 75% Country Day students, as stipulated by Board policy. Leagues and contests will not be allowed to take place, only practices.

Persons wishing to request a practice time may do so for only one season at a time. Practices will not be allowed before or after those dates. The Fall season will be assigned for the dates of August 25 through the beginning of the Thanksgiving holiday. The Winter Season will be assigned for Thanksgiving week through the beginning of Spring Break. The Spring season will be assigned for after Spring Break.

Anyone interested in using the facilities must complete the questionnaire that can be downloaded below and return it to Frank Gendusa, Athletic Director, at fgendusa@fwcds.org. All groups will be required to provide completed Liability Waiver forms and also provide a Certificate of Insurance listing FWCDS as a Named Insured on the policy BEFORE they will be assigned a practice time. After we receive the completed questionnaire, all of the liability releases and the Certificate of Insurance, a time and location will be assigned to you.

Groups are expected to maintain the facilities in the condition in which they were found. Any smaller children who are not participating in the practices but are present on the FWCDS facilities must be supervised at all times, especially in the gymnasiums where equipment has been damaged in the past by outside groups.
